People registered with our practice can now access health and wellbeing coaching; a brand new service available to anyone looking to make positive, lasting change to their health!

What is Health and Wellbeing Coaching?

Busy lifestyles. Long-term health conditions. Difficulties and issues at home. They all impact how our bodies work and feel and are a common reason why people visit their GP. These complex issues can often change how we eat, sleep, feel and stay active; the main building blocks that keep us well.

Pills and medicines can only help so far which is why you might find your healthcare professional recommends you try health and wellbeing coaching; a dynamic, proven approach for lifestyle-related health problems.

A health and wellbeing coach has time to listen and work with you about the things that matter to you. Not just in treatment but in life. At each session, you’ll talk about topics which are important to you, set goals for yourself and be guided towards activities, tools, and things in your life that may help if you need them. Led entirely by you, with your coach right behind you, motivating and encouraging you. Every step of the way.

Benefits of Health and Wellbeing Coaching

· No judgement – our coaches live in the ‘real world’. They understand the real-life issues and challenges most of us face from time to time, better than most. They won’t tell you what to do, judge who you are or tell you to drink avocado smoothies and do yoga everyday either!

· Led entirely by you– you are the expert in your own life. If you want to move forward and improve your physical or emotional health and are ready to commit, our coaches will be right behind you, every step of the way.

· Focused on the practical – the goals you set for yourself are yours alone. Our coaches will help you plan and create manageable steps to reach those health goals and increase your knowledge, skills and confidence along the way.

How to access Health and Wellbeing Coaching

We welcome referrals for patients with long-term lifestyle related health conditions such as Type 2 diabetes, obesity, high blood pressure) who, with support, are willing and eager to make sustainable health behaviour change to improve their own wellbeing. Refer yourself or enquire by emailing wakccg.healthandwellbeingteam@nhs.net One of the health coaches will then contact you for an initial assessment and an informal chat. If you do not have access to email, please ask one of the practice staff to make a referral for you.

Mental Health

There are a range of services out there that can help if you’re feeling stressed, anxious or just not quite yourself.

. The NHS Mental Health website is an excellent place to find information and help.

. Turning Point Talking Therapies are always there to listen and provide support. You can refer yourself if you don’t want to discuss matters with your GP. They also have regular webinars that are free to join if you’re not quite feeling ready for a one-on-one chat, or are just looking for information.

. NightOwls is a confidential support line for children and young people available between 8 pm and 8 am. There is also an online chat option available if you’re more comfortable talking through messages.



. KOOTH offers free, same-day. 24/7 support for 11-25 year old’s (in Wakefield). They are here to provide you with mental health counselling, advice, activities, articles, even podcasts and much more. A young person can sign themselves up anonymously on the website with no criteria to meet and no waiting lists. There is also a live chat messenger option between 12pm – 10pm on weekdays and 6pm-10pm on weekends.

Better Health With the NHS Weight Loss App

Many of us want to start leading a healthier lifestyle. However, the vast amount of information out there can be overwhelming.

Fear not! The free NHS Better Health app can provide you with all the information you need to start making changes in a healthy and, most importantly, achievable way. The app can help you make healthier food choices, set weight loss goals (if that’s your aim), and allow you to get more active and burn more calories.

A 12-week personalised guide will be created for you based on your current BMI and health goals. With this, you will be able to make simple changes to your regular food and exercise regime. Small changes make a big difference and are much more manageable than fad diets or strict exercise regimes you will struggle to maintain. Taking better care of your body can also lead to better mental health as well as physical, it’s not all about focusing on weight loss.

Military Veterans

We are proud to be a veteran-friendly GP practice. Our GP’s / ANP’s are here to provide you and your family with advice and support. There are also outside organisations (such as Help for Heroes) that do amazing work, specifically related to helping military veterans.

Op COURAGE are also here to provide help regarding mental health, or any other military related health issues which you may need support with. With help from the NHS, Op COURAGE have expanded to provide treatment for complex mental and physical trauma, as well as alcohol and substance misuse. As part of this, their care co-ordinators are here to act as a point of contact for veterans, helping you to navigate health and care services and liaising on your behalf with health and care professionals.

Non-NHS Services

Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:

  • Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.).
  • Insurance claim forms.
  • Passport signing.
  • Prescriptions for taking medication abroad.
  • Private sick notes.
  • Vaccination certificates.

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.

Flu Vaccine Clinics

Our Flu Vaccine Clinics have commenced. Please contact the surgery on 01977 644850 to book an appointment, or to arrange a home visit if you are housebound.


Are you Eligible for a Flu Vaccine?

The following groups are eligible for flu vaccination (2019/20) as part of free NHS care:

  • Those aged 65 years and over on or before 31 March 2020.
  • All children aged 2 and 3 years of age on 31 August 2019 (dob between 1/09/2015 and 31/08/2017).
  • All children in primary school.
  • Pregnant women.
  • Carers.
  • People living in long-stay residential care homes or other long-stay facilities.

  • Those aged 6 months to under 65 in clinical risk groups including:
    Chronic long term respiratory disease.
    Chronic obstructive pulmonary disease (COPD) or bronchitis.
    Chronic heart disease such as heart failure.
    Chronic liver disease.
    Chronic kidney disease at stage three, four or five.
    Chronic neurological disease such as Parkinson’s disease, motor neurone disease or learning disability.
    Diabetes.
    Splenic dysfunction.
    A weakened immune system due to disease (such as HIV/AIDS) or treatment (such as cancer), including their household contacts.

Not Eligible for a Flu Vaccine?

Even if you are not eligible for a free flu vaccine under the NHS, you can obtain the flu vaccine from many pharmacies and supermarkets, for a small charge.

We’re not just here for when you are unwell. Our team of healthcare professionals and back-up staff offer a number of clinics and services to promote good health and wellbeing whatever your medical condition.

Other Services

Churchview Health Centre has an on-site pharmacy, optician and various outpatient clinics. Our GP practice also has a physiotherapist, pharmacists and nurses who specialise in diabetes and asthma.

Smoking Cessation Clinic

Our specially trained advisors are available to provide practical help and advice on how to stop smoking. We can refer you to a Stop Smoking service, or you can contact an adviser yourself through the NHS website. You can also call the free Smokefree National Helpline on 0300 123 1044.

Minor Surgery

Minor procedures (such as joint injections) can be performed in the practice by our doctors.
